DATA SUBJECT APPLICATION FORM

1. General Explanations

Pursuant to Article 11 of the Personal Data Protection Law No. 6698 (“Law”), you may apply to Flokser Kimya ve Sanayi Ticaret A.Ş. (“Company”) using this application form to exercise the rights granted to personal data subjects regarding all personal data processed by the Company.

In this context, personal data subjects have the right to apply regarding the following matters:

  • To learn whether personal data is being processed,
  • To request information if personal data has been processed,
  • To learn the purpose of processing personal data and whether they are used in accordance with their purpose,
  • To know the third parties to whom personal data is transferred domestically or abroad,
  • To request correction of personal data if it is processed incompletely or incorrectly,
  • To request the deletion or destruction of personal data within the framework of the prescribed conditions in the event that the reasons requiring its processing cease to exist, despite having been processed in accordance with the provisions of Law No. 6698 and other relevant laws, pursuant to Article 7 of the KVKK,
  • To request notification of the aforementioned correction, deletion, or destruction processes to third parties to whom personal data has been transferred,
  • To object to the occurrence of a result against the person themselves by analyzing the processed data exclusively through automated systems,
  • To demand compensation for damages in case of loss due to unlawful processing of personal data.

Pursuant to the 1st paragraph of Article 13 of the Law, the application must be submitted to us in writing, via e-mail over the internet, or by other methods determined by the Personal Data Protection Board (“Board”).

You must fill out this application form completely and send it to the e-mail address specified above, either in person with a wet signature or by signing it with a “secure electronic signature” as defined in the Electronic Signature Law No. 5070.

The Company will announce how applications will be received after other methods are determined by the Board.

Pursuant to the 2nd paragraph of Article 13 of the Law, your applications submitted to us will be answered as soon as possible and within thirty days at the latest from the date your request reaches us, depending on the nature of the request.

As a rule, requests will be fulfilled free of charge; however, if the fulfillment of the request requires an additional cost, a fee may be requested from you.
